Please have your medical referral letter translated into Japanese or English before your hospital visit

Chiba University Hospital requests that any medical referral letter written in a foreign language, excluding English, be translated into Japanese or English by a recognized translation service before you visit our hospital so we can easily understand information about your condition from your previous doctor. If your referral letter is not translated, you will possibly be unable to consult with a CUH doctor on the day of your hospital visit. The same translation requirement applies to the document detailing your medical information.

The following will not be accepted as a medical referral letter

  • A referral letter addressed to a medical institution other than Chiba University Hospital.
  • A referral letter not addressed to any institution.
  • Images and examination data only.
  • Memos or equivalent from the doctor who recommended CUH.
  • A referral letter from an osteopathic clinic.
  • A referral letter written by a person other than a medical doctor or dentist.

First visit to hospital

Please be sure to bring the following items with you

Reception

Reception and registration take time at the first visit. Please make sure you have plenty of time.

  1. Go to the information desk. Please take a number ticket.
  2. Fill out your registration form. You must fill out all red blocks
    (examination date, name, date of birth, address, telephone number).
  3. When your number is shown on the display board, please go to Counter 1 for First Visitor and Cashier.
  4. After your registration, proceed to your relevant department.
