Information for new patients

- Please note that most departments at CUH require an appointment.
- Before visiting appointment-only departments, please call its outpatient service desk to schedule an appointment.

- The general hours of reception are 8:30 to 10:30 a.m. Your department may accept check in at other hours as well, so please call its outpatient service for details.
Please have your medical referral letter translated into Japanese or English before your hospital visit
Chiba University Hospital requests that any medical referral letter written in a foreign language, excluding English, be translated into Japanese or English by a recognized translation service before you visit our hospital so we can easily understand information about your condition from your previous doctor. If your referral letter is not translated, you will possibly be unable to consult with a CUH doctor on the day of your hospital visit. The same translation requirement applies to the document detailing your medical information.
The following will not be accepted as a medical referral letter
- A referral letter addressed to a medical institution other than Chiba University Hospital.
- A referral letter not addressed to any institution.
- Images and examination data only.
- Memos or equivalent from the doctor who recommended CUH.
- A referral letter from an osteopathic clinic.
- A referral letter written by a person other than a medical doctor or dentist.

First visit to hospital
Please be sure to bring the following items with you
- Health insurance card or My Number Card as a health insurance card

- Drug record notebook (drug administration record written or printed by your pharmacy)
- Common Questionnaire
(If you can download the common questionnaire, please fill out your answers and bring it with you when visiting our hospital.)
Reception
Reception and registration take time at the first visit. Please make sure you have plenty of time.
- Go to the information desk. Please take a number ticket.
- Fill out your registration form. You must fill out all red blocks
(examination date, name, date of birth, address, telephone number). - When your number is shown on the display board, please go to Window 1 for First Visitor and Cashier. If you have My Number Card as your insurance card, please insert the card into a card reader at the counter for confirmation.
- After your registration, proceed to your relevant department.

Re-visit without appointment
If you need examination at a new department, or if your last examination was more than 3 months ago and you do not have an appointment this time,
- Go to the information desk. Please take a number ticket.
- When your number is shown on the display board, please go to Window 1 for First Visitor and Cashier.
- Submit your patient card, health insurance, referral letter and medical record to the receptionist. If you have My Number Card as your insurance card, please insert the card into a card reader at the counter for confirmation.
- After your registration, proceed to your relevant department.
